Case Manager - Crisis Interventionist

Job Description:

Under the general supervision of the crisis supervisor and with some latitude for independent judgment, the Crisis and Intake Specialist assesses crisis situations, obtains and coordinates services for individuals in crisis to ensure the individual’s needs are met.

• Primary responsibilities include screening, crisis intervention, performing comprehensive onsite and mobile crisis outreach assessments, developing a crisis resolution with client, family members, or other interested parties. 
• Making decisions regarding disposition of crisis re: return home, respite, hospitalization, intake and continuity of care.
• Work requires aggressive intervention and frequent in-person contact with individuals, families, and service providers.

Full Time - 40 Hours/Week

Salary: $15.37 - $18.31/Hour (Depends on Experience)
