JOB SUMMARY

Takes all reasonable steps to assure that social services are provided throughout the Center. Coordinates Social Service activities with other departments. Provides social service support to residents and their families and assists in Admissions processes.

JOB QUALIFICATIONS:

Education: 

  • Bachelor’s Degree in Social Work from an accredited college.
  • Personal Job-Related Skills: Effective oral and written communication abilities. Counseling skills.
  • Licensure, Registry or Certification: Licensed by the Texas State Board of Examiners as a Social Worker.


Experience:

Prior Work Experience:

  • One year’s social work experience in health care setting preferred.
  • Technical Training: None required.


Physical and Mental Requirements: 

  • Duties of the position require moderate standing and walking. 
  • Lifting up to a weight of 25 lbs.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

 Provides social service support and services.

    • Provides psycho-social assessments, needs assessments, social histories and participates in interdisciplinary evaluation of individuals' needs for adjustment to placement at the time of their admission.
    • Develops individual social service care plans designed to meet the needs of the resident, the family and significant others.
    • Establishes initial care plan goals that promote adjustment and decrease the trauma of relocation to a new environment.
    • Assist residents, potential residents and families and significant others in finding and utilizing financial, legal, mental and other community resources.
    • Provides information, support service and networking to resident families and significant others concerning changes in residents' condition and life situation.
    • Participates in room change decisions, preparing residents and their families and significant others for change, helping them to adjust to change.
    • Protects and promotes the rights of residents and facilitates regular scheduled Support Meetings for families.
    • Reports significant patient rights issues to the Administrator.
    • Facilitates resident’s integration into the community through discharge planning and follow-up services.
    • Participates in staff orientation and staff in-service to promote an understanding of the psycho-social needs and related issues of residents.
    • Is able to complete admission paperwork with potential residents and/or families if needed. Gives residents and families understanding of the admission process for facility.
    • Serves on the Interdisciplinary Care Plan Team which meets regularly to discuss resident care plans.

Promotes positive relations with the community.

    • Participates in community activities that will help the community’s understanding of psycho-social needs in acute health care and long-term care.
    • Establishes and/or facilitates special interest groups providing socialization and/or support involving residents, family and significant others.

OTHER SIGNIFICANT REQUIREMENTS:

  • Maintains confidentiality while working with confidential matters on a daily basis.
  • Provides service direct to patients while maintaining a positive patient relations atmosphere.
  • Interacts with patients, family members, Medical Staff members and other department managers on a continuous basis.
  • Follows Universal Precautions when involved in direct patient contact or handling of potentially infectious materials.
  • Follows safety guidelines and policies.


Salary: $45,000.00 - $65,000.00/Year (Depending on Experience
